Integrated Caregiver Pathway (ICP) (patent pending)

 

The caregiver assessment Integrated Caregiver Pathway (ICP) (patent pending) is designed to take a complete look at your caregiving needs. The assessment questions address the overall needs of the caregiver as an individual, based on Maslow's Hierarchy of needs. Maslow's Hierarchy of needs is a widely utilized tool that places the needs of each individual in order of importance as they relate to life, love, family and one's personal well being. Maslow believed that basic needs such as food, water and shelter needs must be met first.

 

Please complete and answer the assessment in its entirety. The information you lend will assist us in helping you and supporting you as a caregiver. After submitting the assessment you will be contacted in five to seven business days by a Caregiver Case Manager to develop your personalized Caregiver Careplan.

 

The Caregiver Care Plan is designed to focus on the needs identified in the caregiver assessment. The Caregiver Case Manager will offer options to meet your needs as a caregiver. The information obtained during your assessment and careplanning process is confidential and cannot be shared with anyone without your consent.
